Progressing Disability Services

St. Joseph’s Foundation is the lead agency for two Childrens Disability Network Teams in North Cork. There are 14 CDNT’s in Cork – Kerry community healthcare. We work in partnership with

  • Cope Foundation
  • The Brothers of Charity
  • The HSE
  • CoAction
  • Enable Ireland

We provide supports to children aged from 0 to 18 years with complex needs.

Complex needs refers to one or more impairments which contribute to a range of significant functional difficulties that require the services and support of an interdisciplinary disability team. 

Complex needs will result in a child having moderate or severe difficulty particpating in normal daily living appropriate for their age.

Childrens Disability Network Team (CDNT) 

The Children's Disability Network Team (CDNT) supports children and young people with complex needs. This service is for children who require the supports of an interdisciplinary team. CDNTs operate in defined geographical areas.

Children are assigned to the team that is local to their home address. The CDNT is an interdisciplinary team who work collaboratively with the child and family to identify the needs and priorities of the child and family, plan goals and develop an IFSP. 

Individual Family Service Plan (IFSP)

This is a written plan that focuses on the strengths and needs of the child and family, incorporating their priorities and concerns. The family and the CDNT agree their goals and devise a plan to work towards these.
