Referrals for adults into St. Joseph's Foundation can be submitted directly from a number of sources such as Social Workers, GPs , Public Health Nurses, HSE Disability Offices -CHO3 & CHO4 (Mid-West & South) or directly from an individual/their family/carers.
Once a referral has been received you will be contacted by St. Joseph's Foundation. The Social Worker will arrange a home visit, at which an application form will be completed and the process will begin for acceptance into the services at St. Joseph’s Foundation.
Placement in adult services must be agreed with the relevant HSE area and associated funding approved by the HSE.
